The progressive aging of the population makes it increasingly essential, in advanced stages of illness, for healthcare services to ensure continuous, personalized, and culturally competent care. This dissertation analyzes, through a qualitative Case Study following Yin's methodology (2018), the community and palliative care pathway of M., a patient with advanced oncological disease in comorbidity with HIV infection, of Nigerian origin, followed across three consecutive care settings: an Infectious Diseases ward, an intermediate residential care facility, and the community hospice of the Local Health Authority (AUSL). The aim of the study is to identify critical issues, facilitating factors, and improvement strategies to ensure continuity of care and personalization of treatment, investigating how continuity of care, community-based care, and cultural and spiritual factors influenced the management of M.'s complex palliative needs. Data collection relied exclusively on clinical and care documentation, anonymized in accordance with GDPR, analyzed using a grid adapted from Yin's model and subjected to triangulation. The analysis revealed a delay of approximately two months between the recognition of the palliative need and the formal activation of the Palliative Care service, resulting in an information gap in the handover to the hospice. The comparison between settings showed a marked gap in multidimensional assessment, absent in the hospital setting and structured in the hospice, where it guided an Individualized Care Plan consistent with M.'s wish to "live the days she had left to the fullest." Cultural and spiritual factors emerged as a clinically active variable, capable of affecting adherence, communication, and the relationship with the care team; their unsystematic management generated avoidable tensions, particularly with the caregiver, a dual figure acting as both a relational resource and an obstacle to premedication, comparable to the concept of the "hidden patient." Primary Nursing emerged as the main facilitating factor, having enabled the relational continuity necessary to build trust. An original finding concerns the identification of therapeutic negotiation, interpreted through Horne's concordance model (2006), as an advanced nursing competency capable of mediating the bioethical tension between autonomy and beneficence. Despite the critical issues identified, the pathway concluded with clinically significant comfort, evidenced by a PAINAD score of zero in the final hours of life.
Il progressivo invecchiamento della popolazione rende sempre più centrale, nelle fasi avanzate di malattia, la capacità dei servizi sanitari di garantire una presa in carico continuativa, personalizzata e culturalmente competente. Il presente elaborato analizza, attraverso un Case Study qualitativo secondo Yin (2018), il percorso di presa in carico territoriale e palliativa di M., persona assistita con patologia oncologica avanzata in comorbilità con infezione da HIV, di origine nigeriana, seguita in tre setting consecutivi: un reparto di Malattie Infettive, una struttura socio-assistenziale intermedia e l'Hospice territoriale dell'AUSL di pertinenza. L'obiettivo dello studio è individuare criticità, fattori facilitanti e strategie migliorative per garantire continuità assistenziale e personalizzazione delle cure, indagando in che modo la continuità assistenziale, la presa in carico territoriale e i fattori culturali e spirituali abbiano influenzato la gestione dei bisogni palliativi complessi di M. La raccolta dati si è basata esclusivamente su fonti documentali cliniche e assistenziali, anonimizzate secondo il GDPR, analizzate mediante una griglia adattata dal modello di Yin e sottoposte a triangolazione. L'analisi ha evidenziato un ritardo di circa due mesi tra il riconoscimento del bisogno palliativo e l'attivazione del servizio di Cure Palliative, con conseguente lacuna informativa nell'handover verso l'hospice. Il confronto tra i setting ha mostrato uno scarto netto nella valutazione multidimensionale, assente in ospedale e strutturata in hospice, dove ha orientato un Piano Assistenziale Individualizzato coerente con la volontà di M. di "vivere al meglio i giorni che le rimangono". I fattori culturali e spirituali sono emersi come variabile clinicamente attiva, capace di condizionare aderenza, comunicazione e relazione con l'équipe; la loro gestione non sistematica ha generato tensioni evitabili, soprattutto con il caregiver, figura duale tra risorsa relazionale e ostacolo alla premedicazione, riconducibile al concetto di "paziente nascosto". Il Primary Nursing è emerso come principale fattore facilitante, avendo reso possibile la continuità relazionale necessaria alla costruzione della fiducia. Un risultato originale riguarda l'individuazione della negoziazione terapeutica, letta attraverso il modello di Horne (2006), come competenza infermieristica avanzata capace di mediare la tensione bioetica tra autonomia e beneficenza. Nonostante le criticità, il percorso si è concluso con un comfort clinicamente rilevante, testimoniato da un punteggio PAINAD pari a zero nelle ultime ore di vita. Lo studio dimostra che continuità assistenziale, valutazione multidimensionale strutturata, competenza culturale e continuità relazionale sono condizioni organizzative con ricadute cliniche dirette sulla qualità della presa in carico palliativa in contesti multiculturali, e propone di riconoscere la negoziazione terapeutica come competenza infermieristica avanzata autonoma, da promuovere attraverso formazione e supervisione clinica riflessiva.
